Requirements for PSK Membership, Contribution and Cluster Investment

1. Beneficiary must be a Philhealth member and present a birth or baptismal certificate and if married-a marriage or tribal marriage certificate from NCIP. In case he is not an active Philhealth member, he shall pay the Philhealth charges.
2. Membership fee of P100.00 is paid upon initial application effective for one year.
3. Member has contributed to the cluster investment of P200.00 to RODCH upon initial membership and every year of each membership renewal.

1. No age limit
2. Ward accommodation. In case accommodation is upgraded, beneficiary shall pay the excess of the ward accommodation. Professional fees of non-member physicians as requested by the beneficiary the excess shall be his accountability.
3. The Philhealth then other social service assistance will be deducted from the total charges before the PSK benefits can be utilized.
4. Any member who avails any of the benefits is required to fully pay the total yearly contribution for those on staggered payment basis.
5. Members who contributes on staggered basis shall pay an initial of at least P400.00 which covers membership P100.00 effective for year, cluster investment P200.00 and initial PSK payment-P100.00.
6. Members can avail in other hospitals in cases of emergency/life threatening situation or when RODCH is incapable to render the service.
7. A delinquency of 3 months contribution will disqualify the beneficiary to avail of the benefits. In case payment is updated he shall pay the gap within the year of membership. The effectivity period after one month upon initial membership, upon renewal is not required if renewed within one month after expiration.
8. Only Medicines listed in the Philhealth drug formulary is compensable. Medicines not included shall be paid by the patient; Generic drugs are encouraged to expand the value of the fund.
9. Contributions are evidenced by acknowledgement receipts, or the authenticated passbook of the member. Official receipt is issued upon full payment. Contribution must be payed only through authorized channels or representatives.
10. Payments shall be sent to R.O Diagan Cooperative Hospital PSK office through Kwarta Padala Agencies or directly to R.O Diagan PSK secretariat then shall deposit it to their respective DBP cluster checking account or any other designated bank. Each team shall have an assigned reference number.
11. Signatories of the checking account are the assigned treasurer of the cluster and the PSK CEO.
12. Non availment for the recent year PSK contribution will be added to existing mutual funds owned by the respective cluster which shall be added funds and benefits for the respective cluster. Proceeds for other projects or investment shall be decided by the cluster.
13. Any further excess charges must be the patients counter-part payment
14. Member who have unrenewed PSK membership shall still benefit from their cluster investment and their individual investment.

Exclusion:

• Pre-existing disease; hypertension, diabetes, chronic heart disease, chronic mental illness, chronic arthritis, chronic renal disease, tuberculosis, pre-existing surgical disease or other chronic disease.
• Child birth after 2 children.
• Cosmetic surgery.
• Circumcission & other non-essential procedures.
